Responsibilities:
Provide routine individualized care to the elderly, convalescents, or persons with disabilities Monitor and report changes in patient health status Provide personal care, such as bathing and dressing patients Light meal prepping and homemaking skills Must have drivers license/car to drive patients to appts/shopping needs Teach family members ways to care for patient Understand that our Veterans need stability and that if you commit to them, that you are required to show up for them.

Qualifications:
Experience in hospice, home health, or other related fields Positive and patient demeanor Ability to build rapport with patients Ability to handle physical workload-Must be able to stand, walk, squat, bend, kneel, reach, twist, push, and pull.
Varied assistance is required depending on the mobility of the client.
Must be able to lift 50 pounds Ability to read, write, speak and understand English and communicate effectively State of NH Background check/BEAS check Negative TB skin test or chest x-ray.
Off site testing available.
Valid driver's license in good standing and vehicle insurance are required Obtain Alzheimer/Dementia training (Company Paid) within 60 days High school diploma or GED

This is a 40 hour/week position.
